SV Winter Sports Physicals and Drug Testing

Seneca Valley will hold drug testing and school physicals for winter sports at the senior high school. For drug testing, students must bring a completed Seneca Valley Parental/Guardian/Custodian Drug Testing Consent Form which is available in the office, at www.svsd.net under the athletics tab or in any secondary school office. The $28 fee may be paid in cash or a check payable to SVSD. Winter sports physicals will also be offered to students and athletes will need to submit a PIAA Comprehensive Physical Form. Sections 1, 2 and 3 must be completed by a parent or guardian and the doctor will sign section 4. The $15 fee may be paid in cash or a check payable to SVSD. Forms are available on the internet at www.svsd.net under the athletics tab or in the athletic office. For more information, please visit the above Web site or contact the athletic office at (724) 452-6040, ext. 1752.
